Your Initial Visit

First time clients at Pinewood Natural Health Centre must complete a new patient intake form to give clinic professionals a rapid overview of patient history and symptoms. A unique pediatric intake form is necessary if the patient is 15 years old or younger; a parent or guardian must sign the pediatric form. Clients must also sign the consent forms and a confidentiality form.

To save time, you can download and print out these forms and fill them in before your initial consultation.
